Application

Please complete one application for each child you wish to enroll at The Hope Institute Learning Academy.

If the form is not complete, we will not be able to include your child in the enrollment process. Applications are valid for the 2013-2014 school year only.

You must submit legible copies of the following forms for your application to be complete:

• Student Birth Certificate or Legal Guardianship Papers
• Parent/Guardian Home Utility Bill, Mortgage Bill or Lease to verify Chicago residency
• Parent/Guardian License or I.D. Card
• Completed New Student Application*Please note that you must be a Chicago resident to apply
